New Patients

Thank you for considering Embrace Mental Health & Wellness for your mental health care. We are accepting new patients.

You will receive an invitation from Simple Practice our HIPPA compliant and secure electronic health record, to create an account to complete our new patient screening. It is vital that you complete this screening in its entirety so that we may decide if our services are acceptable for you. Please note that it may take 2-3 business days for a response. You will be notified via your Simple Practice account if your request is accepted or denied.

We will make all attempts to provide patients with referral resources should we not be able to meet their mental health needs.
